A key element of therapy is determining the correct position of the lower jaw within the joints; we call this the central relation (CR)
Treatment for patients who need to replace multiple missing teeth, who wish to improve the aesthetics of their smile using crowns and veneers, as well as patients with worn-down teeth, must be carefully planned. Treatment that alters a patient’s occlusal relationships (the shape and height of the teeth) leads to a different position of the head and jaw in the temporomandibular joints. Most patients are familiar with the feeling of discomfort when a filling is too high. Often, this very slight discrepancy causes significant discomfort and leads to joint pain and even headaches. In the case of a filling, these are fractions of a millimeter; in the case of occlusal reconstruction, the height varies from a few to over a dozen millimeters. This process requires the joint to adapt to the new position of the condyle within the glenoid fossa.
